Eligibility for Continuous Care: Children with Medically Complex Conditions
Children from birth through age 21 with medically complex conditions that require continuos care including but not limited to:
- Cerebral palsy
- Shaken Baby Syndrome
- Muscular dystrophy
- Tracheostomy care
- Chronic asthma
- Apnea monitoring
- Hydrocephalus Microcephalus
- Traumatic Brain Injury
- Spina bifida
- Tube feedings
- Diabetes
- Cystic Fibrosis
- Down syndrome
- Epilepsy
- Congenital heart defects
- Prematurity complications
- Colostomy care
- Stroke
- Other Genetic Anomalies
- Failure to thrive
- Seizure Disorders
- Renal diseases
- Medication Monitoring
- Chronic lung disease
- Oxygen administration
- IV Therapy
STEPS TO ENROLL
STEP ONE
Contact us to schedule an Initial Assessment to determine the eligibility of your child.
STEP TWO
Request the Physician’s order/referral to enroll your child in our program.
STEP THREE
Our staff will complete a Plan of Care according to your child’s medical needs and request authorization for services.
STEP FOUR
Once your child has been enrolled, we will work with you in transporting your special needs child’s to and from our location with our provider-supervised transportation service.
